Palliative Physician & Nurse Practitioner
Our Medical Director, Dr. Thomas Putnam, and Nurse Practitioner, Cynthia Carlson, work closely with the patient’s primary care physician and other specialists, as well as other members of the palliative interdisciplinary team. Dr. Putnam or Ms. Carlson typically visit every patient in their home to assess the patient’s health and circumstances and formulate a care plan. They focus on three goals:
1) Alleviating troubling symptoms (pain, vomiting, depression, etc.).
2) Educating and guiding the patient and families with difficult decisions.
3) Helping coordinate care between doctors and caregivers based on what is most important for the patient.
Dr. Putnam is the only board-certified palliative care physician in Chautauqua County; Ms. Carlson has specialized training and experience in caring for the elderly and seriously disabled, as well as wound care.
A Palliative-certified Registered Nurse Case Manager is responsible for managing the implementation of the patient’s care plan. The nurse makes house calls, is available by phone on a 24/7 basis and plays a large role in monitoring and coordinating the care of the patient.
