Our hospice physicians oversee the interdisciplinary teams that carry out the unique care plan that is established for each patient. They conduct the weekly team meetings where every hospice patient’s care plan is reviewed, as well as consult with team members and the patient’s primary care physician on an ongoing basis whenever changes or concerns about patient care arise. Our Medical Director, Dr. Thomas Putnam, is the only board-certified palliative care physician in Chautauqua County and is widely respected for his specialized training. When it is helpful, the physician may visit hospice patients where they live to better understand and alleviate their suffering.
A Hospice Registered Nurse Case Manager is responsible for admission, management and coordination of all patient care. Under the supervision of our medical director and in concert with Licensed Practical Nurses, they provide compassionate medical bed-side treatment by monitoring symptoms and vital signs, administrating medications, changing dressings and tending to the patient’s unique medical needs.
