Every family confronted with a life-limiting illness faces unique challenges. While there is no formula we can provide to eliminate suffering and pain, there are steps that individuals and families can take that will make the transition less difficult. While we hope the first step that families will take in this journey will be to call Hospice, we have made available the following helpful information for your convenience.
Health Care Proxy
Illness or injury can prevent a patient from consulting with their physician and making their own medical decisions. Appointing someone you trust as your health care agent to make these decisions if you become unable is one way to protect yourself. You have the right to appoint someone as your health care agent by filling out a form called a Health Care Proxy.
